Diabetes specific care in these plans
These plans carry benefits specifically targeted at helping people with their particular condition. Chronic Special Needs Plans (C-SNPs) focused on diabetes offer additional options over non-SNP plans. Some key benefits include capping insulin costs at a lower amount, no cost diabetic supplies through Optum Home Delivery, and unique drug formulates focused on diabetic care.
How to qualify for diabetes focused plans
One important thing to note is that pre-diabetic conditions are not covered by SNPs. You must have a primary physician who can verify your chronic condition. Incidentally, these plans have their own election periods, but can also be enrolled in during general election periods.
After enrollment into a C-SNP plan, you will go through a verification process 2 months post enrollment. Eligibility should be confirmed by the end of the second month of coverage. If you are not verified or some error occurs, you will be entered into a special election period to allow you to choose regular coverage. Attempting to enroll in C-SNPs should not cause you to experience a coverage gap.
Provider’s like UnitedHealthcare will make every attempt to verify coverage eligibility prior to the two month post enrollment period. Likewise, if you use an Agent of Newnan Medicare, expect to receive verification from us when you are accepted or denied. Additionally, if you are denied, we will ensure you remain covered under your desired Medicare Advantage plan.
